Please have your doctor's notes pertaining to the condition that you want evaluated as well as the report from your most recent MRI (within the past 6 months). The doctors at University Hospital ask that you fax this information to them prior to scheduling an appointment. Each doctor looks at the records to determine the urgency for appointments.
